Welcome to Life's Final Chapters, a supportive resource for those navigating the complex terrain of serious illness and end-of-life care. Life is a beautiful journey. Still, in life’s most tender phases, having a companion can make all the difference. That’s why this website exists: to help illuminate the path ahead with support and guidance on palliative care and related needs.

I am not a medical professional, but a fellow traveler, having been connected to end-of-life care in both my personal life and my career. Life’s Final Chapters is not a provider of medical advice, but rather a supportive resource designed to help you understand and navigate the options in advanced-illness care, including palliative care, hospice care, caregiver support, grief support, and advance care planning.

My goal is to empower you and your loved ones with the knowledge needed to make decisions that reflect your values and needs. Here, you’ll find straightforward, understandable resources to support you in advocating for the kind of care you desire and deserve.

What is End-of-Life Care?

End-of-life care is specialized medical care focused on providing relief from the symptoms and stress of a serious illness. Broadly speaking, it is palliative care, which is appropriate at any age and any stage in a serious illness and can be provided alongside curative treatment.

Isn't End-of-Life Care Hospice Care?

End-of-life care encompasses all phases of serious-illness care - including both palliative care and hospice care. While both palliative care and hospice provide comfort, palliative care can begin at diagnosis, and at the same time as curative treatment. Hospice care, which is also part of the spectrum of palliative care, begins after treatment of the illness is stopped and when it is clear that the person is not going to survive the illness.

The mission of Life's Final Chapters is to provide insights into various aspects of palliative care with the aim to enhance your quality of life. You will find information on managing physical symptoms, navigating health care planning, tackling emotional and psychological challenges, and spiritual reflection opportunities, all tailored to support a dignified and meaningful journey.
